Home Health & Hospice Care Partners with Catholic Charities

Home Health & Hospice Care (HHHC), a nonprofit Visiting Nurse Association and hospice in Merrimack, has made arrangements with Trinity Home Care, a private duty agency of Catholic Charities NH, to accept patients of Trinity as Catholic Charities phases out its home care operation. A representative of Home Health & Hospice Care will meet with each patient to discuss their service needs. All of the current employees of Trinity Home Care will be considered for employment and will be transitioning through HHHC’s Human Resources Department. HHHC’s goal will be to have Trinity’s patients continue with the same caregiver whenever possible.

Michael Lehrman, vice president of Catholic Charities Health Services, says, “We are happy to be partnering with Home Health & Hospice Care for the provision of home care and feel our clients and community will benefit from this partnership. We have had a long-term relationship with Home Health & Hospice Care and are confident in their quality and commitment to serve the needs of home care clients.” Catholic Charities is a nonprofit religious organization that owns and operates skilled nursing, assisted living and independent living centers throughout NH.

According to Julie Stone, director of business development for HHHC, the organization anticipates a "seamless and positive experience for patients and families.”

Home Health & Hospice Care serves more than 5,000 patients in southern NH, including greater Manchester and Nashua. The agency provides traditional home care, including skilled nursing, IV therapy, rehabilitation therapies, medical social work, licensed nursing assistants, specialized pediatric care, and advanced illness management.
