Three NH Health Care Organizations Join Forces

Exeter Hospital, Core Physicians, and ClearChoiceMD Urgent Care have formed a clinical affiliation. The relationship currently includes ClearChoiceMD's urgent care centers in Portsmouth and Epping and Core Physicians and Exeter Hospital's new Plaistow Regional Healthcare facility.

The new Plaistow Regional Health facility will have a ClearChoiceMD urgent care center in the same building as Core Physicians' adult, pediatric and specialty practices and Exeter Hospital imaging and physical therapy departments. 

“This clinical affiliation will benefit our patients as we work to enhance our clinical care coordination, patient treatment plans and patient transitions between care settings. It furthers our mission to provide unparalleled service and care to the patients, families and communities we serve,” says Melanie Lanier, DO, Chief Medical Officer for Core Physicians.

The clinical affiliation includes a designated point of contact at ClearChoiceMD to ensure optimal communication and coordination of care for patients as they move between ClearChoiceMD, Exeter Hospital and Core Physicians primary care offices and specialists.

“By partnering with Exeter Hospital and Core Physicians we will be able to exchange information and coordinate follow-up care in a timely manner. We will have access to Exeter’s full suite of advanced services including emergency care, advanced imaging, physical therapy, and Core's multiple subspecialties including orthopedics, neurology, and many others,” says Marcus J. ClearChoiceMD Urgent Care has developed similar relationships with LRGHealthcare in the Lakes Region, and Catholic Medical Center in Manchester.
