Fifth Organization Joins North Country Healthcare Partnership

North Country Home Health & Hospice Agency in Littleton affiliated with North Country Healthcare (NCH), formed in 2016 as partnership between North Country health care providers. With approximately 100 North Country Home Health & Hospice Agency employees, this affiliation brings the total number of people employed by North Country Healthcare to more than 1,400 health care professionals, making NCH one of the largest employers in the North Country. 

“North Country Healthcare is excited to welcome the employees, physicians, nurses, and volunteers from the North Country Home Health & Hospice Agency into our family of caring professionals,” says Warren West, CEO of North Country Healthcare. “Through this affiliation, we will be able to expand the continuum of care for our patients, and have home health services under one roof to support our entire program and the mission of North Country Healthcare.”

In April 2016, four regional hospitals partnered to form NCH, including Androscoggin Valley Hospital in Berlin, Littleton Regional Healthcare, Upper Connecticut Valley Hospital in Colebrook and Weeks Medical Center in Lancaster. The partnership aims to improve access across the system, expand patient care services and develop more opportunities for healthcare professionals.

This month, North Country Home Health & Hospice Agency combined with Northwoods Home Health & Hospice in Lancaster to expand its network of after hospitalization care for local patients and their families. The organizations is also working with the City of Berlin to transition patients and staff from their home health department.
