Easterseals NH Partners with Manchester Hospitals to Support Veterans

Easterseals NH Military Veterans Services (MVS) in partnership with Catholic Medical Center (CMC) and Elliot Health System are partnering on a hospital-based care coordination program to serve patients and employees who are veterans, service members, and family members.

CMC and Elliot Health System, will supplement their current care coordination team with an embedded Easterseals MVS care coordinator at each facility. Services will include, but are not limited to resource referral, substance use disorder treatment and wrap-around care, general care coordination, housing and homelessness, employment, women’s issues, military sexual trauma, aging and senior issues, discharge planning, and care transition.

In addition to services, linkages may also be available to health care, benefits, legal issues, mental health services, counseling, and emotional support resources for individuals, couples, and children. Additionally, emergency financial assistance may be provided through Veterans Count, a fundraising program of Easterseals, that provides critical and timely financial assistance and services when no other resource is available to veterans, service members and their families, to ensure their dignity, health and overall well-being. When assistance is provided, it is done so as a hand-up, not a hand out and in a larger plan for self-sufficiency is developed.

These free, confidential, and local services will be provided to veterans of any era regardless of VA eligibility, discharge status, length of service, or active duty status. Since Easterseals launched the MVS program in 2006, more than 11,280 veterans, service members, and their families have been served. The partnerships with CMC and Elliot Health System will broaden the impact of care coordination across the social determinates of health such as housing, employment and behavioral health.

The MVS Care Coordinator will be on staff three days a week at CMC at 100 McGregor Street in Manchester, and two days a week at the Elliot Hospital main campus at 1 Elliot Way in Manchester. Veterans, service members, or military families who are patients or employees at the CMC or Elliot Health System, who are in need, should contact Leslie at 603-851-0281.
